Your appeal must include:
-Your name
-Your social security number
-Your current address
-Your telephone number
-The decision you want to appeal
-The reason you want to appeal
-Your signature and don't forget to include the date

and copy it and write the date and time you send it in so that you can keep it for your records.
If they say they didn't recieve it.
