There are a number of ways that you can write a request for a new ATM card and pin to your bank. If you have lost your card or pin number you can firstly try writing an email through online banking to make your request.

I wish to request for a new ATM card and pin to be sent to me as I have lost my card. I would appreciate if you could kindly send it to the following address:

You can send this request via email or write it in letter form and submit it personally to your bank who will then be able to start the process of initiating a new card and pin number.
Most banks take around 5-7 working days to send you a new ATM card and pin number. You may receive a quicker response from your bank if you visit them personally or send an email message through online banking.
