First of all, write the designation of band manger with his bank name and complete address. Then make a subject line where you will have to write the reason of your letter in brief.

Use some salutations and make your first paragraph by introducing yourself and then go on to ask them to issue you with a cheque book. Supply your bank account details, then finally sign your letter at the end with your name and complete address.

You don't write a letter for a check book. Instead, you can call your bank and speak with someone to order one.  I order my checks from Designer Checks online - no problem.  They contact
your bank to verify your account number and their routing number.  Checks in the Mail is another good company.  They both cost less than what the banks would charge.
