If you can't read and write, can someone - like mother or girlfriend - give you some help? This seems to be what you are asking because filling out a form is really nothing more than reading the instructions and anwering in appropriate blanks. The form will ask such things as name of the work prospect and possibly the interviewer, how you made contact (whether with application, resume, cold call, or interview, etc), the prospect company's name and contact number (so that your submitted responses can be verified) . . . Simple things like that. I assume you have made notes or recorded those contacts (or had a friend help you do so) or you might be in for a problem or have your benefits delayed or denied. But what you really need is someone there to help you; not someone "everywhere and nowhere" like us visiting a Web site.
